MHSAA- Physical Form
All students must have an up to date physical on file with the athletic office in order to participate in any given season of a sport (up to date is considered AFTER April 15th of the previous school year). Between you, your student athlete, and your practitioner, please make sure all fields are completely and accurately filled out.

In some exceptional cases, a parent/guardian may wish to request a waiver of the school provided transportation to and from athletic contests provided to their student athlete. Waivers are for a case-by-case basis and must be presented to the coach at least 2 days in advance.
