Election to Transport Student
If you want to take your child home from an away event instead of taking the bus this form must be filled out.
Transportation Consent From
This needs to be filled out in order for a parent to take students home from a game.
Physical Form
If you have turned in physicals for another sport please let Coach Curmi know. If you do not, they must be turned in to Coach Curmi before tryouts. You cannot tryout if these forms are not turned in.
Emergency Card
Must be updated every year for every season and sport.
